Glandular fever (also known as infectious mononucleosis) is an infection most commonly caused by the Epstein-Barr virus (EBV), which is a human herpes virus. Glandular fever is not particularly contagious. It is spread mainly through contact with saliva, such as through kissing, or sharing food and drink utensils.
"Mono" or infectious mononucleosis is characterized by fever, sore throat, and testing may be indicated for abnormal liver function or low platelet counts
There are specific test that can detect antibodies to the infection. This is the best test. While a complete blood count (CBC) may show some changes consistent with glandular fever it is not specific. Patients with glandular fever are diagnosed by their symptoms and the findings of a full blood count (FBC) and a monospot test (which tests for a heterophile antibody). A certain percentage of those who have glandular fever will have a negative mono test; this is especially true with children.

The thrombocytopenia was thought to be due to a factor in the blood that agglutinated group-0 platelets to a high titre. But among 57 patients with glandular fever without purpura, 24 had platelet counts below the author's lower limit of normal (140, 000/c.mm.). Nearly all the low counts were found in the 2nd and 3rd weeks of the disease; the lowest figure was 63, 000/c.mm. Severe thrombocythaemia, when it occurs, is probably an extension of this tendency, of which the causation is unknown.
